We generally ask that patients arrive 15 minutes prior to their initial evaluation to complete the necessary paperwork. If you would prefer to complete the paperwork before your appointment, these forms can be printed at your convenience.

Patient Referrals
We believe that the best way to promote our services is by personal recommendations from our patients! To show our appreciation for recommending us to help someone else that you care about, we give $15 Hannaford gift cards for every family/friend referral.
At Head to Toe PT, we pride ourselves on ‘Treating the individual, not just the injury’; ensuring that our patients obtain their optimal health is our main focus. The next time you encounter someone that our services might help, mention Head to Toe PT and if they become a patient and mention your name, we’ll get a gift card right out to you.
Frequently Asked Questions
Do I need a referral from my doctor?
Not necessarily. In Maine, physical therapists have what is called “direct access.” This basically allows individuals to seek care without a doctor’s referral. However, some insurances require a referral, regardless. If you are unsure, give us a call. We are happy to give your insurance provider a call to find out what you need.
How do I know if physical therapy is covered under my insurance?
Prior to your first appointment we will call your insurance company to verify benefits so there are no surprises. Remember that your insurance is your responsibility. Therefore, we highly recommend calling your insurance company personally to double check your benefits and get answers to any questions you may have.
I don’t have insurance. Can I still come in for physical therapy?
Yes, of course! We have a cash price for our services, and we’re more than happy to work out a payment plan if necessary.
My doctor referred me to a different PT clinic, but I want to see you. Can I?
Of course! In the state of Maine you have the right to choose where you go for your physical therapy treatment. Simply inform your doctor that you would prefer to visit Head to Toe PT.
What should I expect at my first appointment?
Your first appointment will last about an hour. You will meet your physical therapist and have an evaluation, where your PT will assess your pain, physical situation and diagnose the problem. From there, a plan will be created to help you return back to wellness through home exercises and visits to the clinic.
What do I need to bring to my first appointment?
Bring your insurance card and any paperwork you have received from your doctor. Also, if you take medication, it may be helpful to bring a list of those with you. If you have not pre-printed and filled out the required paperwork from our website (see patient forms section), please show up at least 15 minutes early to allow for enough time to complete your paperwork.
What type of clothing should I wear?
Usually loose fitting clothing such as sweatpants and a t-shirt are best. If you are coming in for your knee, please bring or wear shorts.
Will I be seeing a physical therapist or an assistant?
At Head to Toe PT, we hire only licensed physical therapists – no assistants or aides. We make every effort possible to ensure you see the same PT at each of your appointments.
How many times a week will I need to come?
That will depend on the plan of care determined by you and your physical therapist. Typically patients come 2-3 times each week.
Do you have late appointments?
Yes we do! And early ones, too! We see patients as late as 6:30 pm and as early as 7:00 am depending on the day of the week.
What if I can’t make it to my appointment?
If you cannot make it to your appointment, please call to reschedule as soon as possible so we can put someone else in your spot.
Do you accept debit and credit cards?
Yes we do. In addition to cash or check, we accept Visa and MasterCard.
I was injured at work. Can I still come see you?
Yes. While your employer may recommend a particular physical therapy clinic, it is ultimately up to you to choose where you go for treatment. Just be sure to inform us ahead of time so we can verify coverage of your claim.
I have been referred for aquatic therapy. What can I expect?
Your first appointment will be on land, in the clinic so that we can perform an evaluation; you will not be in the pool that day. After your initial appointment, you will be in the pool one-on-one with your therapist. Please remember to bring pool clothes with you for all pool appointments. There is a locker room with showers on-site for changing before and after your appointment.
